Ensure the manufacturer is listed as “RUO.bio,” and include the product name and batch number (e.g., BP5001). Request a sample photo from the facility for documentation.
2. Document Shipment Preparation: Photograph and document the unopened product(s) before shipping. All images must clearly display the batch number and vial cap color. A pre-shipment photo is REQUIRED. Note: Any evidence of tampering will invalidate eligibility for a rebate.
3. Submit Results: Direct the testing facility to send a copy of the results to [email protected].
4. Complete Verification Form: Upon receiving the results, submit the contact form below with:
   • A link to the test results.
   • The pre-shipment photo(s).
